Employees of Alameda County Medical Center may make their Angio gift through a regular payroll deduction. Just download the Employee Payroll Deduction form here. Complete the form, sign and date it, make a copy for your records, and return the form to us by postal mail or fax. We will forward your request to ACMC Payroll.
